Project PHASES is Community Rehabilitation Center’s (CRC) dedicated initiative to combat the spread of HIV in Jacksonville through a "Whole Person" approach to sexual health. We believe that by removing barriers to testing and providing non-judgmental education, we can empower our community to take control of their status and their future. We offer: Rapid & Confidential Testing: Get accurate results in a supportive, private setting. Physician-Led Linkage to Care: If a test is reactive, you aren't alone. We provide immediate, direct connection to our care physicians and specialized clinical teams to begin treatment planning. PrEP & PEP Education: We offer the latest information on preventative medications to keep you and your partners safe.
